How to register as a patient

Go to the Register with a GP Online Service and follow the on screen instructions.

If you do not wish to Register Online you can download and complete the following form:

  • Register with a GP Surgery (PRF1) form
  • You do not need photo identification to register, but will need to provide this if you wish to set up online access to your medical record.

When you have completed the form visit reception in person with it.

The Doctors are happy to see patients who are temporarily resident in the Practice area should medical advice be required. In this instance persons wishing to be treated should state that they are a temporary resident when reporting to reception.

To register you will need to bring your passport and complete our Temporary Resident Registration form (also available from reception).

New Registration Health Check

You will be asked to make an appointment with the Health Care Assistant who will ask you one or two details about your past health and present life style. This will benefit both the practice and yourself as it can take some time for your notes to be received from your former GP.

Accountable Named GP for All Patients

When registering with Rising Brook Surgery, you register with the practice, granting you the flexibility to consult with any of our clinicians. If you prefer to see a specific clinician, please inform us when booking your appointment. Kindly note that this may result in a longer waiting time. For continuity of care, all clinicians have access to your medical records.
